Intrafraction motion during partial breast irradiation depends on treatment time

چکیده
Abstract Introduction As the prognosis of early-stage breast cancer patients is excellent, prevention radiation-induced toxicity has become crucial. Reduction margins compensating for intrafraction motion reduces non-target dose. We assessed tumor bed throughout APBI treatment fractions and calculated CTV-PTV breathing drift. Methods This prospective clinical trial included treated with on a Cyberknife fiducial tracking. Paired orthogonal kV images made entire fraction were used to extract position. The modelling calculate amplitudes. needed compensate drift according Engelsman Van Herk respectively. Results Twenty-two patients, 110 5087 image pairs analyzed. 0.3–0.6 mm. margin increased time after first imaging positioning. For total duration up 8 min, 1.0 mm sufficient. 32 min, 2.5 mm needed. Techniques that account can reduce by 0.1 mm. There was systematic trend in caudal, medial posterior direction. To this, 0.7 mm could be added margins. Conclusions longer due drifting target. It doubled 24 min compared 8 min. Breathing limited effect.

Real-Time Intrafraction Motion Management During Radiotherapy
Real-time intra-fraction target tracking is a technique that enables adjustment of the target radiation while it is being delivered (i.e., intra-fraction adjustments) to compensate for movement of the organ inside the body. Real-time tracking, which may or may not use radiographic images, is one of many techniques referred to as “image-guided radiotherapy” (IGRT). For this policy, realtime trac...
متن کاملQuantifying the effect of intrafraction motion during breast IMRT planning and dose delivery.
Respiratory motion during intensity modulated radiation therapy (IMRT) causes two types of problems. First, the clinical target volume (CTV) to planning target volume (PTV) margin needed to account for respiratory motion means that the lung and heart dose is higher than would occur in the absence of such motion. Second, because respiratory motion is not synchronized with multileaf collimator (M...
متن کاملPartial breast irradiation for early breast cancer.
BACKGROUND Breast conserving therapy for women with breast cancer consists of local excision of the tumour (achieving clear margins) followed by radiation therapy (RT). RT is given to sterilize tumour cells that may remain after surgery to decrease the risk of local tumour recurrence. Most true recurrences occur in the same quadrant as the original tumour. Whole breast RT may not protect agains...
